Behavioral support includes mental health and substance use treatment and rehabilitation services. Dedicated Home Health Services is striving to achieve a designation in Behavioral Home Health Services by ACHC. Behavioral Home Health is an innovative and integrated healthcare. It is a recovery-oriented and people-centered service that promises better patient experience and better outcomes than those achieved in traditional home health or outpatient programs.

The DHHS Behavioral Home Health Service delivery model provides cost-effective, longitudinal “homebound” care. It facilitates access to an inter-disciplinary team to provide behavioral health care, medical care, and community-based social services and supports for both adults and children with chronic conditions. 

Components of the Behavioral Home Health Services include:

• Skilled Psychiatric Nurse Assessment, medication reconciliation, and follow-up. This includes monitoring medication side effects, action, and interactions, as well as providing basic reinforcement of behavior modification principals established by the treating psychiatrist. Some counseling can be given by the psychiatric nurse in reinforcement of the psychiatrist’s behavior plan;
• Comprehensive care management;
• Care coordination;
• Health promotion;
• Comprehensive transitional care;
• Individual and family support;
• Referral to community and support services; and
• Health information technology.

Our goal is to improve the individual experience of care, improve the population’s medical, mental, and spiritual health, and reduce healthcare costs. DHHS will:

• Implement and evaluate the Behavioral Home Health delivery model as a way to achieve accessible, high-quality, and integrated health care;
• Demonstrate cost-effectiveness in order to justify and support the sustainability and spread of the model;
• Support behavioral health practices by increasing available home health, primary and co-morbid disease care resources;
• Enhancing care coordination to result in improved quality of care and patient outcomes; and
• Decrease unnecessary inpatient hospitalization and emergency room visits.

Speech therapy is provided for disorders of communication, cognition, and swallowing. When speech intervention is necessary, it means that one of the most essential human functions is compromised. Our Speech-language pathologists are trained professionals that provide the expertise and services:

Here at DHHS, our experienced physical therapists can help improve mobility, function, and motion, as well as effectively manage pain after an illness, injury, or surgery. Your therapy will be focused on helping you restore normal mobility so you can get back to the things that you enjoy. Our physical therapist comes to your home and provides you with high-quality rehabilitation services.

Occupational therapy (OT) is the use of assessment and intervention to develop, recover, or maintain meaningful activities, or occupations. Our Occupational Therapists help patients resume their typical, everyday lives and regain their independence. Their experience includes helping people with behavioral health problems, disabilities, injuries, or impairments.
